How thin does a dress watch need to be?

Under about 11 mm slides under any shirt cuff. Between 11 and 12 mm works with most. Above 12 mm the watch will catch, whatever the dial looks like.

Can a dress watch have a date window?

Traditionally no, because it breaks the symmetry of the dial. In practice most affordable dress watches have one, and a colour-matched date wheel makes it far less intrusive.
